New Mexico State Process

 Before you start your application, make sure you have the following: 

  • A clear (face visible) copy of your current New Mexico Driver’s License or New Mexico photo ID. Temporary New Mexico Driver’s License and photo IDs are acceptable.
  •  A copy of a one-page clinic note related to qualifying condition (see application for details). 


1. Submit an official patient application.


This application is the same for first time and returning patients and there is no charge to apply or to renew a patient ID card.

  • Complete the Patient Information: the patient should make sure all the information is correct.
  • The patient must include a clear (face visible) copy of your current New Mexico Driver’s License or New Mexico photo ID. Temporary New Mexico Driver’s License and photo IDs are acceptable.
  • Include a copy of a one-page clinic note related to qualifying condition (see application for details).
  • The application form must be dated and have ORIGINAL signatures by both the patient and your Leafwell medical provider. These cannot be photocopied.
    • If the patient is 18 years old or older and the form is signed by someone else, please send a completed Medical Power of Attorney or Legal Guardianship paperwork to indicate legal authority.
    • For any patient under 18 years old the following must also be included:
      • A Caregiver Application with all required documents completed by a Parent or Guardian; and
      • A copy of the patient’s birth certificate.
  • If you are submitting your annual medical certification and need a new card, please also complete and send an Information Change/Replacement Card form because a new card will not be automatically issued for the annual medical certification. 


2. Once you have completed the application, please mail or drop off to the below address.

3. After you apply 

  • The Medical Cannabis Program may take up to 30 days from the date a complete application is received at the MCP office to approve or deny the application  An additional 5 business days are allowed to process and issue the registry identification cards.
    • It is recommended that applications be submitted 45 to 60 days before expiration to avoid problems obtaining or possessing medical cannabis.

Note that each card is valid for 3 years but you must re-certify with a Leafwell physician every year to provide proof that your medical condition still qualifies you as a medical card holder.
