Oregon State Process

Congratulations on getting approved for your medical certification. Here's how to complete the state application process once you have your Attending Provider’s Statement from Leafwell. You must do this within 90 days of your appointment with Leafwell.

Getting Your Oregon Medical Marijuana Card

1. Create a new account on the Oregon Medical Marijuana Online System - https://ommpsystem.oregon.gov
   1. You will need to confirm your email address.
2. Click Register as a Patient.
3. Click New Patient.
4. Submit all the information as required and click Next.
5. Verify that the address is correct (they will mail you the physical card) and click Yes or No to correct the address.
6. You can designate a caregiver if you wish.
7. You can also designate a grower.
   1. Growers must be 21 years or older.
   2. You must provide the grow site address.
8. Submit your physician's information - this will all be found on the Attending Provider’s Statement emailed to you after your appointment with Leafwell. 
9. You then choose which fee you will pay.
   1. There is the option to upload documents which allow you to pay reduced fees if applicable.
10. Upload the documents as required. These documents may be:
    1. Government-issued ID
    2. Proof of residency (ie. recent utility bill or rental agreement)
    3. Your Attending Provider's Statement (dated within 90 days)
    4. Caregiver government-issued ID (if applicable)
    5. Grower's government-issued ID (if applicable) 
    6. Proof of grow site zoning (if applicable)
    7. Documents that prove eligibility to lower fees (if applicable)
11. Complete this application by ticking the Electronic Signature and Certification boxes and typing your name into the box.
12. Before submitting, review all information entered to check it is correct.
13. Pay the fee.
14. You will receive an email once the payment has been received and processed. This can take up to 2 business days. 
    1. At that time you can print out a 30-day receipt which permits access to a dispensary for 30 days while you wait for your state card to arrive via US Mail.
